We will gladly fill out routine health, immunization, and other forms at no cost to you, as long as your child is up to date on immunizations and routine check-ups. We will do our best to have these forms ready within 5 business days. Please allow time for us to complete these forms correctly and accurately. Keep in mind that forms brought on the same day as well check-ups are usually completed during that visit.
IF YOU NEED A FORM COMPLETED IMMEDIATELY, we can provide expedited service for a $10 fee, when possible. We will do our best to get expedited forms ready within 24 hours (excluding weekends and holidays.)
Unlike many practices, at Pediatric Physicians PC we do not charge for routine forms, letters, or copies of medical records. It’s our pleasure to help.
